Creatine has a branding problem. For decades it has been sold as a gym supplement, packaged in tubs covered in muscle imagery, and shelved alongside pre-workouts and mass gainers. That positioning is correct as far as it goes. Creatine is one of the few performance supplements with a five-decade research base for strength, power output, and recovery. It works, and it works for almost everyone who trains.

What the positioning misses is everything else creatine does. The cognitive research is now substantial. The mood research is emerging. The data on older adults, on vegetarian and vegan diets, and on women across the menstrual cycle, all support a much wider use case than the gym subset.

If you have never considered creatine because you do not lift weights, this is the case for taking it anyway.

What creatine actually is

Creatine is a small molecule the body makes from three amino acids, stored mostly in muscle tissue, used as a rapid recycling system for ATP. ATP is the body's energy currency at the cellular level. Creatine donates a phosphate group to regenerate spent ATP, particularly in tissues with high and bursty energy demand: muscle, brain, the heart.

The body makes about half the creatine it uses, with the other half coming from diet, mostly from red meat and fish. Vegetarian and vegan diets are well below the dietary intake of omnivores, which is one reason the strongest supplemental response is often seen in those groups. Pretty much everyone is operating below maximal creatine saturation without supplementation.

The cognitive benefits

The brain consumes roughly 20 percent of total body energy. Tissues that consume energy heavily benefit from a more efficient ATP recycling system, which is why the cognitive research on creatine has built up so quickly over the past decade.

The most robust findings are in conditions that stress brain energy metabolism. Sleep deprivation is the cleanest example. People who are sleep-deprived and supplementing creatine perform better on cognitive tasks than sleep-deprived controls. The effect size is modest but consistent, and it shows up in working memory, reaction time, and mental fatigue scores.

In healthy, well-rested adults, the cognitive effect is smaller and more situational. Vegetarians and vegans show a clearer cognitive lift than omnivores, which fits the dietary baseline argument. Older adults show clear improvements in memory and processing speed. Younger omnivores show small effects on average, larger effects under cognitive stress.

The clinical picture for mild cognitive impairment and early-stage dementia is now substantial enough that creatine is being studied as a low-cost adjunct in trial settings. The same data is interesting for healthy adults thinking about long-term brain health.

Muscle and recovery

The strength research is the most-established. Roughly five years of creatine supplementation, in randomised trials, produces about 5 to 15 percent greater strength gains and slightly greater muscle mass gains compared to controls. The effect is reliable enough that creatine is the default supplement recommendation for almost anyone who trains.

The recovery side of this matters even for people who do not lift. Faster recovery between sessions means better adherence to whatever movement practice you do, from running to yoga to walking briskly. The bone-density research, particularly in postmenopausal women combining creatine with resistance training, is also encouraging.

Who should consider it

The categories most often missing creatine but most likely to benefit:

Office workers and knowledge workers. Cognitive demand without high physical demand. Particularly relevant for people doing complex mental work under chronic mild sleep restriction, which describes most knowledge workers.

Older adults. The age-related decline in muscle mass and cognitive performance are both areas where the creatine evidence is now solid. Pairing supplementation with a basic resistance-training routine is one of the highest-leverage interventions in the literature.

Women, particularly across the menstrual cycle. The female-specific creatine research has caught up rapidly in the past five years. There is now solid data on cycle-related cognitive and mood effects, and on training response, that supports daily use.

Vegetarians and vegans. The dietary baseline is lower, so the supplemental response is typically larger. Cognitive and physical effects both tend to show up more clearly.

The dose that actually works

The dose with consistent evidence is five grams per day of creatine monohydrate, taken at any time, with or without food. The old protocol of loading at 20 grams a day for a week is unnecessary. Five grams daily saturates the muscle stores over three to four weeks, which is the timeframe over which effects emerge anyway.

Common concerns

Water retention. Real, but modest, and confined to the muscle cells where the creatine is stored. Not the bloating people imagine.

Kidney safety. Studied extensively in healthy adults. No issue. People with pre-existing kidney disease should discuss with their clinician.

Hair loss. A single study from 2009 found a small increase in dihydrotestosterone. The finding has not been replicated and no follow-up has shown an actual hair-loss effect. The concern is over-stated relative to the evidence.

The bigger framing

Creatine is the most-researched supplement in existence by a margin. It is cheap, well-tolerated, and the effect size on the metrics that matter (strength, cognition, recovery) is consistent enough that it sits in a small group of supplements where the burden of evidence has effectively shifted to the people not taking it.
